The Free Health Care Center at Catholic Charities thrives thanks to the generous work of our community. We are always welcoming those who would like to volunteer their medical expertise—both hands-on with patients and administrative duties—as well as non-clinical volunteers to support our office, events, and special projects.

Care Navigator / Social Work Volunteer

We are seeking a dedicated volunteer trained in social services or case management to serve as the main point-person for patient follow-up. This role is essential in helping patients navigate the healthcare system and access the resources they need.

Key Responsibilities
  • Assist patients with financial assistance applications and charity care programs
  • Schedule specialty referrals, imaging, and preventive services
  • Help with transportation, language needs, and paperwork
  • Track completion of referrals, tests, and next steps
  • Identify and maintain up-to-date community resource lists
Qualifications

This role can be filled by:

  • A licensed social worker, OR
  • A trained volunteer with experience in patient advocacy, case management, or community health
